Healthcare in 3D Terms and Phrases

  • Patient — Individual Provider Acts upon.
  • Patient Centered Medical Home — Is a methodology to provide primary comprehensive patient centered care, where a single physician or “medical home” is responsible for coordinating the patients care throughout the patient journey. In the context of HCn3D it is the observation and influence the medical home is accountable for across all network entities providing care.
  • Patient Centered Network — Hierarchal network where the lowest level is centered at the patient (Patient > Provider > Provider Group or Association > Network Neighborhood > Plan > Region > CMS and multistate insurance plans > Population).
  • Patient Centering — Positioning the cube at the patient level. Specifically the inflection point where the provider action is decided. At this lowest level, centering is on this single point along a patient’s care path.
  • Patient Journey — The patient’s care path through the network entities.
  • Remote Patient Monitoring (RPM) — B In its simplest form, RPM involves the use of connected electronic tools to record personal health and medical data in one location that is reviewed by a provider at a different location. The data may or may not be viewed as soon as it is transmitted. RPM can be used to treat both chronic and acute conditions, enabling clinicians to keep tabs on patients in-between clinic visits or when in-person care is not possible. — mHealthIntelligence | Accessed June 23, 2022
